Qualifying for a Virginia Medical Marijuana Card
Virginia State Qualification Requirements:
- Patient must be a resident of Virginia and will need to provide proof of residency.
- Patient must provide proof of identity using either an identification card or a valid Driver’s License issued by the state of Virginia, or a valid Passport issued by the U.S. Government.
- Patient must be diagnosed with a debilitating condition or disease determined by the licensed practitioner, where CBD or THC-A medical marijuana could alleviate symptoms.
- Patients must be issued a written certification by a licensed Physician, Nurse Practitioner or Physician Assistant who is registered with and certified by Virginia’s Board of Pharmacy to recommend CBD or THC-A Oil.

- Amyotrophic Lateral Sclerosis (ALS)
- Ulcerative Colitis
- Cerebral Palsy
- Cystic Fibrosis
- Terminal Illness Requiring End-Of-Life Care
- Fibromyalgia
- Rheumatoid Arthritis
- Post Herpetic Neuralgia
- Headache Syndromes, Migraine headaches
- Neuropathic Pain
- Muscular Dystrophy
- Anxiety/depression/mood disorders
- Interstitial Cystitis
- MALS Syndrome (Median Arcuate Ligament Syndrome)
- Vulvodynia and Vulvar Burning
- Tourette Syndrome
- Chronic Pain of at least 6 months duration
- Other medical conditions as determined by our practitioner
